BIRTH CERTIFICATE
We need a copy of the student’s birth certificate
CITIZENSHIP DOCUMENT
If the student is born outside of Canada, we need a copy of their proof of citizenship.
INTERVIEW
To see if we are a good fit for your family, we like to meet with prospective students.
ENTRANCE EXAM
To better prepare for your child’s education, Lady Fatima Academy requires students to do an entrance exam.
BUS FORM
Fill this form if you need a bus service to pick up and drop off your child. We have routes available for all quadrants of Calgary.
RELEASE FORM
This form needs to be signed to release your child from their previous school.
REGISTRATION FEES
A non-refundable registration fee of $300 needs to be paid for your child to have a spot booked for the next year.
